CHI INNOVATE 2019
Hundreds of professionals from the health sector have gathered at the CHI Innovate 2019 international congress, held in Singapore, on May 9 and 10. In his presentation 'From hospital to Population Health Management', Manuel Bosch, has presented the population health management model developed by
Shore Health. A program that tries to efficiently manage the health needs of the population. It is a predictive, personalized, preventive and participatory assistance model.
For Bosch, "Current management models are oriented towards the treatment of acute patients and their inadequacy for chronic patients implies an increase in costs and poorer quality of care". In this sense, as he has qualified, Ribera Salud is incorporating new tools capable of helping to transform the public health system. One of the priorities is the advance towards the digital era, she has remarked, "Incorporating a more continuous control of patients, helping them to be co-responsible in the management and control of their diseases."
It is not the first time that the health group has been invited to present the keys to its management in international forums. It should be remembered that the Ribera Salud model is a case study at Harvard University and the University of Berkeley, among other organizations, and that it was selected at the WISH International Innovation Forum in Qatar as one of the seven global success stories.
