    Ribera Salud Hospitals receive an award for their care program for patients with heart failure

    III Edition of the Quality, Sustainability and Innovation Awards from the Ad Qualitatem Foundation

    • The Hospitals of Torrevieja and Vinalopó have received the national award for the best project in Patient Care Programs by the Ad Qualitatem Foundation.
    • Both centers have obtained this award thanks to their remote patient management program and structured comprehensive care for patients with chronic heart failure.

    The University Hospitals of Torrevieja and Vinalopó, managed by the Ribera Salud group, have received first prize in the fourth category for the best project executed in Patient Care programs by the Ad Qualitatem Foundation, recognition of the most humanizing innovation in healthcare, in the third edition of the Quality, Sustainability and Innovation awards.

    This event is intended for initiatives and projects that stand out in the field of quality promotion. In this sense, both centers have obtained the award for their remote patient management and structured comprehensive care for patients with chronic heart failure, chosen as the most valued initiative.

    It is a care plan for the chronic patient of the Ribera Health group that proposes the development and implementation of a remote management system for patients with this pathology and that allows to improve their quality of life and adapts the use of health resources.

    The Ribera Salud group will donate the financial amount of the prize, 6.000 euros, to social purposes.

    About the Care Program for patients with heart failure

    The Torrevieja University Hospital and the Vinalopo University Hospital, managed by the Ribera Salud group, have an innovative project called "Program With You", in which 55 complex chronic patients who receive help to better understand and monitor their disease and improve their quality of life participate.

    These patients have been provided with the necessary technology (tablet, blood pressure monitor and scale with bluetooth system), with the aim of improving the control and follow-up of their disease and analyzing the influence of remote management control on the number of exacerbations, hospitalizations and Mortality of patients with Heart Failure (CHF).

    During the first two weeks, the nurse makes a weekly phone call to the patient in order to find out about possible incidents and the patient's adaptation to the program, from there, and while they are included in the program, a videoconference is held with the patient every 15 days. Once the follow-up period (6 months) is over, it is assessed whether the patient should continue follow-up for the Home Hospitalization Unit (UHD) or if referred to the Primary Care case management area.

    The program has been very well received by patients, who have indicated that they are satisfied with the initiative. In this sense, despite being unfamiliar with new technologies, they state that they have no difficulties handling them.
