These awards acknowledge the work of professionals, institutions, and companies whose digital initiatives have added value, contributed to the sustainability of the system, and improved patient life throughout 2024 and 2025.
The objective of this project was to create and validate an algorithm to help predict the 30-day readmission risk for any major deprivation clinic (MDC) and the corresponding tools to optimize patient care according to their actual needs. It has proven successful in facilitating the work of healthcare professionals and the resources used, enabling proactive rather than reactive support.
The project to “Identify patients at risk of readmission within 30 days,” developed by the technology company of the Ribera healthcare group, has won an award in the AI and data management category at the fifth edition of the Ennova Health 2025 awards. The ceremony was held at the Real Teatro de Retiro in Madrid. The award was accepted by Mireia Ladios, Product Manager at Futurs.
