The aim of this plan is to improve communication between primary care physicians and pharmacists to achieve optimal care for chronic patients. The MEDAFAR project is promoted by the Spanish Society of Primary Care Physicians (SEMERGEN), the Pharmaceutical Care Foundation and ESTEVE.
